Member Rights & Responsibilities
As an Optum Specialty Pharmacy patient, you have the right to:
- Have personal health information shared with the patient management program only in accordance with state and federal law
- Identify the program’s staff members, their job title, and to speak with a staff member’s supervisor if requested
- Speak to a health professional
- Receive information about the clinical management program
- Decline participation or opt out, at any point in time
Member Rights & Responsibilities
As an Optum Specialty Pharmacy patient, you are responsible for:
- Providing the right clinical & contact information, and disclosing any changes
- Telling your treating prescriber about being in the clinical management program
